End-user and stockist undertaking (EUSU) form
Exporters must submit a completed end-user and stockist undertaking (EUSU) form when they apply for a standard individual export licence (SIEL) or a standard individual trade control licence (SITCL).

Details
The overseas end-user or consignee receiving the items must complete the EUSU form.
From July 2025, the EUSU form should be used for all exports, even those going to stockists.
The UK exporter must use the information in the EUSU form when completing their online SIEL or application.
